Dealer Application Options



DEALER DIRECT APPLICANTS MUST:
1- Be a Musical Instrument Retailer.
2- Provide resale certificate or resale license.
3- Submit 3 credit references in the music industry.
4- Meet minimum order and annual purchase requirements.
5- There are separate application forms for established music retailers (Who have been in business for 5 years or more) and new music retailers (who have been in business for less than 5 years) and all Internet music retailers.

There are two account application options below:

1- You may fill out a form and 'click' submit. Your application is sent to us using the internet. You MUST complete the 'Phone Number', 'email', and 'Name' fields.

Form A- For Established Music Stores (5 or more years in business)

Form B- For New Music Stores (less than 5 years in business)

Form B- For Internet Only Sales

2- Download pdf file. This form must be filled out and faxed or mailed back to us. (People who do not like to submit sensitive information over the internet may prefer this option)
